Please be advised that if you wish to include your academic achievements in your Diploma Supplement, follow the steps below:
- Download the achievement declaration template.
- Complete the form accurately, reflecting your actual achievements.
- Send the completed form in .docx format to stncmumk@gmail.com
Subject line: Supplement [First Name and Last Name] - Once the document is verified by the STN, a signed scan will be sent back to you via e-mail.
If you require an original paper copy, please state this clearly in your e-mail. Paper originals can be collected at the STN office during office hours.

Achievements which can be included in the diploma supplement:
Each achievement must be documented. Scanned copies of certificates or proofs of achievement must be attached to the email along with the supplement template. Entries will NOT be approved without proper documentation.
- Publications in scientific journals indexed in the list of the Ministry of Science and Higher Education with DOI number.
- Authorship or co-authorship of a scientific monograph.
- Awards and honorary mentions for publications presented at scientific conferences.
- Authorship or co-authorship of a patent.
- Participation in research projects receiving grants.
- Other significant scientific awards and distinctions.
