If you wish to include your academic achievements in the supplement to your diploma, follow the steps below:
- Download the achievement declaration template
- Fill in the required information and print the form
- Submit two copies of the form to the office of the Student Scientific Society (STN) during duty hours or leave them at the reception of Student House no. 2
- Wait for approval and collect the document
Achievements which can be included in the diploma supplement:
- Publications in scientific journals indexed in the list of the Ministry of Science and Higher Education
- Authorship or co-authorship of a scientific monograph
- Awards and honorary mentions for publications presented at scientific conferences
- Authorship or co-authorship of a patent
- Participation in research projects receiving grants
- Other awards and distinctions
For each achievement, a copy of the document confirming it (certificate, diploma, statement, etc.) must be submitted together with the achievement declaration form
